Predictors of Acute Phase Response after Intravenous Zoledronic Acid in Patients with Osteoporotic Fracture

Abstract
Objective : Acute phase response (APR) is the most common adverse event of the intravenous zoledronic acid, and can reduce the compliance of annual injection. The purpose of this study is to determine the predictor of APR after 5
mg zoledronate in patients with osteoporotic fractures.

Methods: In this prospective study, patients admitted with osteoporotic fracture, > 50 years of age, and who were
willing to receive zoledronic acid during hospital stay were eligible. During hospital stay, we prospectively evaluated
APR, including fever, flu-like symptoms, headache, arthralgia, and myalgia, after 5 mg zoledronate. We evaluated the
predictors in demographic data by multivariate analysis.

Results: In this study of 159 patients with osteoporotic fractures, the frequency of APR, fever was 32.1%, and 22.0%. The significant predictor of APR were higher bone mass index (BMI) and male gender in multivariate analysis,
especially in terms of fever.

Conclusion: This study showed that predictors of APR after intravenous zoledronic acid are male gender and higher
BMI. Although those were uncorrectable factors before intravenous administration, these concerns on the APR such
as fever can reassure the predictive patients, and minimize compliance decreases.
